There's a burning sensation in your mouth even though you haven't had anything hot to eat or drink. It's an experience you've had for years, often accompanied by mouth dryness, tingling or numbness that leaves you irritable, anxious or depressed.
The root causes for Burning Mouth Syndrome (BMS) remain elusive, although there appear to be links to diabetes, acid reflux, menopausal hormonal changes or even psychological issues. Although we may not be able to pinpoint the root cause we can identify contributing factors to BMS through a detailed oral examination and medical history (including drugs you're taking).
Mouth dryness is one of the most common factors for BMS. The lack of lubrication from adequate saliva flow can contribute substantially to the irritating burning sensation. There are a number of causes for mouth dryness, including as a side effect from many medications or other treatments.
We must also consider whether an allergic reaction — the body's over-reaction to a foreign substance — may have a role in your symptoms. Some people react to sodium lauryl sulfate, a foaming agent found in many types of toothpaste, along with whitening substances or flavorings like cinnamon; denture wearers can become allergic to the plastic materials used to construct the denture. These, as well as spicy foods, smoking or alcohol, can irritate or cause the tissues lining the inside of the mouth to peel.
Determining what factors contribute to your symptoms allows us to develop a treatment approach tailored to your situation. If, for example, we've determined your BMS stems from dry mouth as a side effect to medication, we can ask your doctor to prescribe an alternative, increase your water intake when taking pills or stimulate saliva flow. If we identify an allergen as a factor, you can eliminate the substance to reduce symptoms.
You may also need to make changes to your eating and lifestyle habits: stop smoking, reduce your alcohol or coffee consumption and avoid very hot or spicy foods. And look for ways to reduce stress, another contributing factor, through relaxation techniques, exercise or support groups.
It's possible that BMS will resolve itself over time. In the meantime, though, we can help you find ways to alleviate the irritation.

“Less is more” is a truism for much of life. It’s also an important feature of an emerging approach to treating tooth decay known as minimally invasive dentistry (MID).
MID updates another revolution in dental care that occurred in the early 1900s. Treating decay took a quantum leap thanks to techniques developed by Dr. G. V. Black, considered the father of modern dentistry. Dr. Black’s approach (known as “extension for prevention”) involved not only removing decayed tooth structure, but also adjacent areas deemed vulnerable to decay, which made them easier to clean. On the downside, though, it also created larger than normal fillings.
As the practice prevailed through much of the Twentieth Century another weakness became apparent—the approach could not guarantee a treated tooth would not experience decay again. This became the real impetus toward MID—to find more comprehensive ways to treat decay with as little impact on the tooth structure as possible.
These efforts received a real boost from emerging technology. This was especially true in diagnostics with the rise of new devices like intraoral cameras and techniques like laser fluorescence that can enable dentists to detect decay much earlier. It’s now possible to catch the disease at an earlier stage before substantial damage to the tooth occurs.
MID has also led to new treatments that preserve more of the tooth structure. Traditional drilling is increasingly giving way to air abrasion, the use of a fine particle stream of aluminum oxide, glass beads or baking soda directed precisely at decayed structure and minimizing damage to healthy structure. We’re also using new filling materials like composite resin for restorations after treatment that are strong yet still life-like and attractive.
We also can’t forget the role of the twin daily hygiene practices brushing and flossing to remove bacterial plaque, the main source of dental disease. And regular dental cleanings and checkups round out the MID approach, helping to ensure that decay doesn’t get too far. The end result of this revolutionary approach: your teeth can experience less impact from treatment and remain healthier and more attractive in the long-run.

While it may not be one of your favorite features in the dental office, the dental drill is nevertheless necessary for treating problem teeth. It’s used primarily for removing decayed or damaged structure and preparing a tooth for fillings or other restorations.
Dental drills have been used for decades and are quite effective — but they have their drawbacks. Their rotating burrs often remove portions of healthy tooth structure along with decayed material. Friction from the drill action can cause discomfort, so local anesthesia is usually needed. Drills can also emit a high-pitched machine noise that’s unsettling to many patients.
There’s a growing alternative to the drill, known as air abrasion. Although the technology has been around since the 1950s, the development of new suction pumps that capture the resulting dust from its use has made it more palatable as an option to the traditional drill.
Also known as particle abrasion, the technique uses a pressurized stream of fine particles (usually aluminum oxide, an abrasive powder) directed at teeth to wear away (abrade) the tooth’s structural surface. We can be quite precise in the amount of surface material removed, so it’s useful for diminishing stains or roughing the surface for bonding materials like composite resin. We’re also able to remove decayed material with very little impact on surrounding healthy structure, and you may not need anesthesia during the procedure.
While this quiet alternative to the noisier drill is quite versatile, it does have its limitations. It’s not that efficient for preparing larger cavities for restoration or for removing older amalgam fillings. The teeth to be treated must be carefully isolated to prevent the fine particle dust produced from being swallowed by the patient or spread into the air. High-volume suction equipment is a must or the procedure will create a “sandstorm” of particles in the room.
Still, for situations suited to it and with proper isolation measures, air abrasion can be effective and comfortable. If the technology continues to improve, the dental drill may soon become a relic of the past.
